Consultation Appointment
Thank you so much for choosing us for your oral surgery needs! We would like to welcome you to our practice and we look forward to providing you with the highest quality of care. Our patients are our number one priority and we make every effort to provide you with an excellent experience.
Your initial appointment with us will be a consultation visit, at which time you will get to meet Drs. Mynsberge, Ruggles or Yang and discuss your health history, diagnosis, and treatment/anesthesia options. Although surgery can occasionally be performed the same day as the consultation, most patients will require a second appointment to provide treatment on another day. Please see below as to how to best prepare for this appointment.

Please come prepared with the following information:
- Your completed patient registration forms, which can be accessed by CLICKING HERE.
- This document may be completed and submitted electronically through our secure HIPAA compliant online server (just make sure to click “Submit”) or printed, completed by hand, and brought to your appointment.
- Your dental and medical insurance information (please bring any available insurance cards so that we can make copies), so that we may help you process your claims
- A thorough list of your medical conditions, past surgeries, allergies, and medications you are presently taking, as these can sometimes significantly influence surgical and anesthesia planning

Minors
Any patient under the age of 18 years old must be accompanied by either a parent or legal guardian at consultation and surgery appointments.
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to your appointment. If additional imaging is necessary, it can be taken at our office.
