Your First Visit
Your initial appointment will consist of a consultation appointment, at which time you will get to meet Dr. Mynsberge or Dr. Ruggles and discuss your health history, diagnosis, and treatment/anesthesia options. Although surgery can occasionally be performed the same day as the consultation, most patients will require a second appointment to provide treatment on another day. All patients interested in undergoing IV anesthesia for surgery will need to proceed with their treatment at a separate appointment.
In order to assist us, please be prepared with the following at the time of your consultation:
- Your completed patient registration forms, which may be printed, completed by hand, and brought to your appointment or electronically submitted through the following secure online form.
- Your dental and medical insurance information, so that we may help you process your claims
- A thorough list of your medical conditions, past surgeries, allergies, and medications you are presently taking, as these can sometimes significantly influence surgical and anesthesia planning
Any patient under the age of 18 years old must be accompanied by either a parent or legal guardian at consultation and surgery appointments.
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to your appointment. If additional imaging is necessary, it can be taken at our office.
